Abstract: Objective To explore the clinical experience and feasibility of the single direction single port video-assisted thoracoscopic lobectomy for lung cancer radical resection. Methods 131 cases with lung cancer were performed single direction single port VATS lung cancer radical resection. Operation time, bleeding volume, the lymph nodes dissection number, the occurrence of complications, drainage time, and hospitalization stay were recorded to observe the clinical results. Results 131 cases of lung cancer successfully completed operation including 3 cases of right middle-lower lobectomy, but there were 6 cases converted to small incision operation because of vascular injury. It showed operation time of (156.8±30.5) min, the amount of bleeding of (101.2±23.3) ml, drainage time of (5.1±0.7) d, hospitalization time of (7.5±2.6) d, and the lymph nodes dissection number of (13.7±2.9) /case. It was found 23 cases (17.6%) with lymph node metastasis. There were 97 cases of adenocarcinoma, 31 cases of squamous cell carcinoma, and 3 cases of adenosquamous carcinoma. The lung cancer was classified as TNM stage Ⅰa,Ⅰb,Ⅱa,Ⅱb,Ⅲa of 11, 36, 40, 31 and 13 cases respectively. There was no severe complication, nor was the perioperative death observed. Conclusions Single direction single port video-assisted thoracoscopic surgery combines reasonably with two opinions of single direction and single utility port, which is proved to be safe, effective and more minimally invasive.
