| ESD术后追加胃切除术对早期胃癌患者预后的影响 |

摘要 目的 探讨早期胃癌患者前期是否行ESD术对后期胃切除手术的疗效及预后的影响。方法 回顾性分析我院自2011年1月~2014年2月间诊治的早期胃癌患者60例,其中28例行内镜黏膜下剥离术(endoscopic submucosal dissection,ESD)后追加胃切除术,而32例行单纯根治性胃切除术,记录两组住院时间、术中出血总量及住院总费用;记录两组术中淋巴结的清扫方式、清扫数目以及手术总时间,术后发热或感染例数;观察两组术后1年内并发症发生情况,并记录两组术后3年内复发情况及3年存活率。结果 术后ESD追加胃切除组除住院时间和住院费用较单纯胃切除组显著增多,差异有统计学意义(P<0.05)外,其他指标两组均无明显差异。结论 对于早期胃癌患者,在符合ESD适应证的前提下行ESD诊疗,并不会对后期行根治性胃切除的预后产生不良影响,可以作为一种将早期胃癌的诊断和分段治疗相结合的诊疗模式。
