Abstract: ObjectiveTo compare the efficacy and feasibility of miniinvasive intervertebral diskoscopy and traditional open surgery for gluteal muscle contracture(GMC).MethodsThe clinical data of 73 cases of bilateral GMC from June 2003 to June 2012 were retrospectively analyzed. Of which 38 cases were treated with minimally invasive intervertebral diskoscopy(as the diskoscopy group), and 35 cases were treated by traditional open surgery (as the open operation group). The operation time, the length of the incision, the amount of intraoperative bleeding, the postoperative complications and the time of hospitalization were compared between the two groups. The curative effect of the two groups was evaluated, including the gait, the leg test, the leg squat and the jumping sensation were compared.ResultsAll patients were followed up for 13 to 36 months with an average of 18.3 months. The length of the surgical incision was shorter, the amount of bleeding was less and the hospital stays time after the operation was shorter in the diskoscopy group than those in the open operation group, and the differences were statistically (P<0.05), but there was no significant difference in operation time between the two groups(P>0.05). No nerve injuries were found in both groups. One case (2.6%) was found incision hematoma in the diskoscopy group, and was cured by compression bandage and sand bag oppression. There were 6 cases (17.1%) of incision hematoma in open operation group, including 3 cases of incision infection, and cured by dressing change and drainage. There was a statistical difference in the incidence of postoperative complications between the two groups (P<0.05). No significant difference appeared in the curative effect between the two groups 1 years after operation (P>0.05).ConclusionMinimally invasive intervertebral diskoscopy is reliable and effective for GMC with minitrauma, less intraoperative bleeding, fewer postoperative complications and faster recovery.
